CLIENT TERMS
By booking an appointment with Jenna Turner Permanent Cosmetics, you are agreeing to the following terms:
I can confirm that I am over the age of 18, am not under the influence of drugs or alcohol, am not pregnant or nursing and desire to receive the indicated semi-permanent pigmentation procedure.
To my knowledge, I do not have any physical, mental or medical impairment or disability that might affect my well being as a direct or indirect result of my decision to have the procedure done at this time.
I agree to follow all pre-procedure and post-procedure instructions as provided and explained to me by the technician. Failure to do so may jeopardize my chances for a successful procedure and/or result in the treatment being cancelled and charged in full.
I agree to complete the health form at least 3 days ahead of the treatment to ensure it can go ahead. I understand that a failure to complete this form within this time frame means that the treatment may be unable to go ahead, if there are any contraindications or health concerns that have not been disclosed ahead of this time. I understand I will be responsible for paying for the treatment in full and the treatment may not be able to go ahead, as it will be outside of the cancellation window.
I understand there is a possibility of an allergic reaction to numbing agent and/or pigments. A sensitivity test is offered however it does not ensure a client will not have an allergic reaction, now or years later. If waived, you agree to release the technician from liability if you develop an allergic reaction to the pigment.
If an unforeseen condition arises in the course of the procedure, I authorize my therapist to use his/her professional judgement to decide what he/she feels is necessary under the given circumstances.
I accept the responsibility for determining the colour, shape and position of the permanent makeup procedure as agreed during consultation.
I fully understand and accept that non-toxic pigments are used during the procedure and that the result achieved may fade over a period of 1-3 years. Even once the colour fades, pigment itself may stay in the skin indefinitely.
I have been informed that the highest standards of hygiene are met and that sterile, disposable needles and pigment containers are used for each individual client, procedure and visit.
I understand and accept that each procedure is a process requiring multiple applications of pigment to achieve desired results and that 100% success cannot be guaranteed during the first procedure. I understand that I may have to return for a repeat procedure. The result of the procedure can be affected by the following: medication, skin characteristics (dry, oily, sun-damaged thick or thin skin type), personal pH balance of your skin, alcohol intake and smoking, post procedure after care. In some cases, a second top up appointment may be required - this is not included and will need to be booked in the same as the first top up, unless otherwise agreed by the artist.
Upon completion of the procedure there might be swelling and redness of the skin, which will subside within 1-4 days. In some cases, bruising may occur. You may resume normal activities following the procedure, however, using cosmetics, excessive perspiration and exposure to the sun should be limited until the skin has fully healed. Please see after care instructions for more details. The procedure results will look acceptable for you to appear in public without additional make-up. You have been advised that the true colour will be seen 6 weeks after each procedure, and that the pigment may vary according to skin tones, skin type, age and skin condition. You understand that some skin types accept pigment more readily and no guarantee on exact colour can be given.
I have been informed of the nature, risks, and possible complications and consequences of permanent skin pigmentation. I understand the permanent skin pigmentation procedure carries with it known and unknown complications and consequences associated with this type of cosmetic procedure, including but not limited to: infection, scarring, inconsistent colour, and spreading, fanning or fading of pigments. I understand the actual colour of the pigment may be modified slightly, due to the tone and colour of my skin.
I fully understand this is a tattoo process and therefore not an exact science but an art.
I understand that if I have any skin treatments, injectables, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to your permanent makeup procedure. You acknowledge some of these potential adverse changes may not be correctable.
BOOKING, DEPOSITS & CANCELLATIONS
A deposit is required to secure your booking. This amount will be deducted from the cost of your treatment and the remaining balance will be due at your appointment. All deposits are non-refundable. However, we are happy to reschedule your appointment if you provide more than 48 hours’ notice.
If you need to cancel or reschedule, please do so at least 48 hours before your appointment. Cancellations made with less than 48 hours’ notice will be charged 50% of the treatment price and a new deposit will be required to make another booking. As a small business, each appointment blocks out a significant portion of our working day and we may also incur premises costs, even if the treatment does not go ahead.
Please arrive on time for your appointment. If you are running late, kindly inform us as soon as possible. We recommend allowing extra time for traffic, parking and locating the premises.
If you arrive more than 15 minutes late, we may need to cancel your appointment to ensure there is sufficient time to carry out your treatment safely and to the required standard. In this situation, the full treatment fee will still apply and a new deposit will be required to make another booking.
TOUCH-UPS & ANNUAL COLOUR BOOSTS
Your first touch up must be booked within 6–12 weeks of your initial treatment. If you are outside of this timeframe, you will need to book an Interim Top Up (3–10 months) instead.
Typically, only one touch up is required after your initial treatment. However, some clients may need a second touch up to perfect their results, as the skin is a living canvas and healing can vary. We will always advise if this is more likely (for example, very oily skin), but healing outcomes can never be guaranteed due to factors beyond our control. If additional touch-up appointments are required, there will be an additional charge.
Annual colour boosts should be booked before your previous permanent makeup has faded too much. Timeframes vary from person to person, but if the area needs to be fully remapped, it will be treated as a new procedure. Permanent makeup longevity varies depending on lifestyle, skin type, medications, and other factors. We generally recommend an Annual Colour Boost at 12–18 months, but please send a photo before booking so we can confirm you are ready.
If your permanent makeup has completely faded, your appointment may need to be booked as a new treatment
Touch up and annual colour boost appointments are for existing clients only. If your permanent makeup was done by another artist, please get in touch first so I can assess the work and provide a quote.
